I, the undersigned, am writing to confirm that I am the Next of Kin (NOK) of the deceased individual named below. As I am currently unable to report the death in person or digitally, I hereby authorize Eternal Horizon by Silent Tribute Embalming (UEN: 53520036J) and its designated representatives to act as my lawful proxy.

They are fully authorized to submit the required documentation and report the overseas death of my family member to the Registry of Births & Deaths via the official ICA portal.
