I, the undersigned, confirm that I am the Next of Kin (NOK) / legally authorised representative of the deceased named below. As I am unable to attend to the necessary arrangements in person, I hereby authorise Eternal Horizon by Silent Tribute Embalming (UEN: 53520036J) and its designated representatives to act as my lawful proxy to:
  • Collect and transport the human remains;
  • Perform embalming / shower / dressing of the deceased;
  • Apply for the Burial / Cremation permit;
  • Handle all relevant NEA documentation and permits.
